FAQ on filling out the Request for Service –
Information Form
Q: Is there a fee for a member to become connected to
Kan-ed?
A: No membership fees are charged by Kan-ed. However, in order
to become connected, you will incur fees from your local service
provider for connectivity, equipment, etc.
Q: Who would I list as the “Administrative Contact”?
A: Usually, this is the person actually requesting the service
to be initiated, and is someone that has budget authority.
Note: this person may also be the Technical Contact.
Q: In the “Technical Contact” area, please
note that if this is a person or a company that you’ve
contracted with for Technical Support Services, then we need
their information.
Q: Who would I list as the “Other Contact”?
A: This would be a place for you to list your Administrative
Assistant or Secretary – this person would serve as
a secondary contact point for us in the event we were unable
to reach you.
Q: What do I fill in under “Membership Affiliation”?
A: The following are the affiliations we are requesting:
• If you are a member of a hospital “network”,

Q: What do I describe under the “Describe your
LAN” box under Local Network?
A: Describe
• the equipment you’re currently using –

proxies, traffic shaping tools, dial-up services, etc.
• current connection speeds within the LAN and to your
ISP
• infrastructure configuration
Q: What is meant by “Service Providers –
Please indicate all that apply”?
A: Because Kan-ed works with a variety of service providers,
we ask that you list all telecommunications service providers
that offer services in your area. Some areas have only one
provider, but many areas have more than one service provider
in their area. Examples include Rural Telephone, SBC, United
Telephone, etc.
